What is AWANA?
AWANA is a worldwide nonprofit ministry focused on providing Bible-based evangelism and discipleship to children and stands for Approved Workmen Are Not Ashamed (2 Timothy 2:15). The key focus of this ministry revolves around scripture memorization, understanding and application. Parents/Guardians play a key role in helping children to learn God’s Word as they work through the handbooks at home each week. Parents are also welcome to attend any/every night with their child. We appreciate your support and pray that your family will be blessed as a result. 
Cubbies
Ages 4-K. Through songs, games, crafts and story time, Cubbies will learn core truths about God, Jesus, the Bible, and salvation. 
Sparks
Grades 1-2. Sparkies will have their curiosities "sparked" while building a solid foundation for knowing Jesus Christ.
Truth & Training (T&T)
Grades 3-6. T&T kids will discover the meaning of grace and how it impacts their relationship with God and others.
Trek
Grades 7-8. Our middle school students will be challenged to solidify what they believe and embrace their faith.

I understand that this form covers all Quail Lake Community Church sponsored activities.

1. I understand that my child/children may participate in physical activities (i.e., bounce house, waterslide, etc.). As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Quail Lake Community Church, AWANA Clubs International, and any person(s) involved in Quail Lake Community Church’s Children and Youth Ministries.
2. In the event of an emergency that requires medical treatment every effort will be made to contact me or my emergency contact. If I cannot be reached, I give permission to Quail Lake Community Church to secure the services of medical professionals. I assume responsibility for all such expenses.
3. I give permission for my child to appear in a photo or video, which may be taken during AWANA to appear on Quail Lake Community Church’s website, Facebook, Instagram, or other social media platforms, or be used for publicity or display purposes. Note: no identifying or private information will be displayed publicly.
4. Some activities will require an additional permission slip. I will be made aware of all activities through email, the church website, social media, and/or my child.
5. Dress Code: Clothing should be age appropriate, properly fitted, and suitable for a church environment. When the body is standing straight, clothing must cover the chest, back, torso, stomach, and lower extremities from arm to arm to mid-thigh with no exposed undergarments. Shoes must be always worn for safety. Swimwear (when activity permits) must be modest.
6. I have read and agree to the terms and conditions stated above.
